*Form 31-014 Sales tax exemption certificate must be completed and filed with the seller (Homemakers). The company completing must have a valid and active sales tax permit for the items being purchased. If property or services purchased for resale or processing are used or disposed of by the purchaser in a nonexempt manner, the purchaser is then responsible for the tax. Once we have verified the information filed, we will apply it to the company’s account. All purchases that are tax exempt have to be by the holder of the sales tax permit. It can not be applied to a third party purchase.
