Official Contest Rules and Regulations

1. How to Participate

1.1 Contest Period:

Starting at 12:00 a.m. (CST), May 9th, 2023, to 11:59 p.m. (CST), May 21st, 2023, subject to these Official Contest Rules and Regulations, customers receiving a survey invitation from Qualtrics on behalf of Homemakers may elect to be entered into the Homemakers Customer Benchmarking Survey Contest ("Contest") upon completion of the survey.

1.2 Contest Entry and Entry Limits:

NO PURCHASE NECESSARY. To enter the Contest, entrants must complete the survey by 11:59 p.m. (CST), May 21st, 2023. One entry per customer account is allowed for the Contest. Entrants must complete the survey and then opt into the Contest. Once opted in, the entrant will be required to provide the following contact information: full name, mailing address (including house number, street, city, state and zip code), telephone number (including area code), and valid email address.

Upon completion of the survey you will be entered for ONE (1) chance to win a prize. Only entries made or received before the end of the Contest Period will be eligible to win a prize (as defined in Section 2, paragraph 2.1 & 2.2). By entering the Contest, you represent and warrant that you have read and accepted these Rules and Regulations and understand that you may be contacted by the Contest Administrator. The use of any device and/or software to automate the entry process is prohibited.

1.3 Eligibility:

Open only to permanent, legal United States residents who, as of the start of the Contest Period, are physically residing in Iowa and at least 18 years of age. Officers, directors, and employees of Homemakers, members of these persons' immediate families (spouses and/or parents, children, and siblings, and each of their respective spouses, regardless of where they reside), persons living in the same households as these persons (whether or not related thereto), sales representatives who sell to Homemakers, marketing affiliates and partners of Homemakers, distributors to Homemakers, advertising and promotion agencies that work with Homemakers, and any other entity involved in the development, administration, promotion, or implementation of the Sweepstakes are not eligible to enter contest or claim a prize are not eligible to enter or win the Sweepstakes.

1.4 Awarding of Prize:

Subject to the Official Contest Rules and Regulations, THREE (3) winning entrants will be determined by a random drawing from among all eligible entries submitted during the Contest period. Odds of winning depend upon the number of eligible entries received.

2. Prizes

2.1 Prize:

There will be a total of THREE (3) Prizes ("Prize") in the amount of a $500.00 Homemakers Gift card to be won during the Contest Period. The Prize must be accepted as awarded. The drawing will be conducted under the supervision of the Sponsor whose decisions are final in all matters relating to the Contest. No substitution or transfers of the Prize will be allowed. The Prize will be awarded within FOUR (4) weeks of the Prize draw.

2.2 Prize Draw:

A random drawing for the Prize will take place on May 24th, 2023. All of the eligible and valid entries received during the applicable Contest period set out above will be subject to a computerized random draw to select the potential winners. Immediately after Contest draw, the Sponsor will contact the potential winners by return e-mail at the account/address used to enter the Contest which must still be valid or by telephone. The potential winners will have SEVEN (7) business days after being contacted by e-mail or telephone (“Contact Deadline”) to contact Sponsor by e-mail (or telephone) using the e-mail address [or telephone number] provided. Winners are responsible for payment of all federal, state and local taxes on the prize and for any other expenses associated with the prize not specifically described herein. Except where prohibited by law, the potential winner(s) will be awarded his or her prize only after such winners’ eligibility has been verified. The potential prize winners may be required to sign and return (1) an affidavit of eligibility and release of liability, and (2) a Publicity Release (“Prize Claim Documents”). Failure by the potential winner to return such the Prize Claim Documents within ten (10) days after request by the Sponsor may result in disqualification of the potential winner. Should the potential winner not reply to Sponsor by e-mail or telephone by the end of the Contact Deadline, or potential winner does not otherwise comply with the Contest requirements, that potential winner will be deemed to have forfeited any rights to his or her Prize and Sponsor will have the right to select an alternate potential winner from among the remaining eligible entries and contact such persons in the manner provided above except that the alternative potential winner will be given a new Contact Deadline which will be SEVEN (7) business days after contact is made by Sponsor. Before being declared a winner, a selected entry must be verified by Sponsor. Sponsor reserves the right to require proof of age and compliance with these Official Contest Rules and Regulations prior to awarding any Prize. Winners agree to accept their Prize as awarded, and the Prize is not transferable or convertible to cash or otherwise. Sponsor retains the right, in its absolute discretion, to make substitutions of equivalent kind or value in the event of the unavailability of the Prize for any reason. The decisions of Sponsor on all issues relating to the Contest will be final and binding on all entrants.

2.3 Odds:

The odds of being randomly selected for a potentially winning depends on the total number of correctly completed eligible and valid surveys received during the Contest period.

3. General Rules

3.1

All Prize claims are subject to verification by the Sponsor and/or its designated representatives.

3.2

All surveys and entries become the property of Homemakers. Neither Homemakers nor any organization involved in organizing the Contest shall be responsible for any Contest documentation that is unreadable, illegible, incomplete, lost, stolen, misdirected, not received, destroyed, or delayed for any reason whatsoever.

3.3

The Sponsor is not responsible for typographical or other errors in the printing, the offering or the administration of the Contest, or in the announcement of prizes. The Sponsor is not responsible for lost, late, misdirected, undeliverable or incomplete entries due to system errors or failures, or faulty transmissions or other telecommunications malfunctions and/or entries not received resulting from any hardware or software failures of any kind, lost or unavailable network connections, or failed, incomplete or garbled computer or telephone transmissions, typographical or system errors and failures, or faulty transmissions, or for any problems or technical malfunction(s) of any telephone network or lines, computer online systems, servers or providers, computer equipment, or entries not received on account of technical problems or traffic congestion on the Internet, or at a web site, or any combination thereof, including any injury or damage to entrants or any other persons computer related to or resulting from entering the Contest or downloading any material for the Contest, or other telecommunications malfunctions which may limit an entrants ability to participate. By participating in the Contest, entrants agree that the Sponsor and their affiliate companies, advertising and promotions agencies, any entities providing prizes, and all of their agents, directors, officers, representatives, shareholders and employees will have no liability whatsoever for, and the winners will indemnify, defend and hold the foregoing parties harmless from, any claims, injuries, losses, or damages of any kind resulting from participation in the Contest or acceptance, possession or use of any prize that may be awarded. Entrants hereby waive any right to claim ambiguity in the Contest or these official rules. Homemakers is also not liable for any damage or loss that may be caused directly or indirectly, in whole or in part, by the downloading of any software or form or the transmission of any information with regard to entering the Contest.

3.4

Homemakers reserves the right to cancel, terminate, or suspend this Contest, at its sole discretion, in the event a computer virus or bug, unauthorized human intervention, or any other occurrence beyond their control which corrupts or adversely affects the administration, security, or normal operation of the Contest. In the event the Contest is compromised by unauthorized human intervention, tampering, mechanical or electronic failures, or other causes beyond the reasonable control of the Sponsor which corrupt or impair the administration, security, fairness or proper operation of the Contest, the Sponsor reserves the right in its sole discretion to void any entries submitted fraudulently or to suspend, modify or terminate the Contest, in which case the Sponsor shall not be required to award any prize or substitute prize. The Sponsor will prosecute any and all fraudulent activities to the fullest extent of the law.

3.5

By entering, Contest entrants agree to abide by the Official Contest Rules and Regulations and the decisions of Homemakers, which are final. Homemakers reserves the right to change or alter any of the Official Contest Rules and Regulations or to cancel the Contest at any time. Except as specifically provided in the Official Contest Rules and Regulations, the Prize must be accepted as awarded, and is not transferable or convertible to cash. By accepting the Prize, the winners agree to the use of their name, address and/or photograph, without payment, in any subsequent publicity or advertising carried out by Homemakers and/or its advertising agents.

3.6

By participating in the Contest, the winners grant the Sponsor and its licensees, affiliates and assigns the right to print, publish, broadcast and use, worldwide in any media now known or hereafter developed, at any time or times, such winners’ name, portrait, picture, voice, likeness and biographical information as news or information and for advertising and promotional purposes relating to this Contest without additional financial or other compensation, unless prohibited by law. Winners must sign a publicity release confirming such consent prior to acceptance of the prize unless prohibited by law. Winners further agree to appear for any presentation or other activity which may include filming, audio, video, electronic or other recordings and/or interviews as may be determined from time to time by the Sponsor unless prohibited by law. By providing information pursuant to this Contest, the entrant consents to the Sponsors use of such information for promotional or any other purposes allowed by law.
